HIV Non-Medical Case Manager
The Non-Medical HIV Case Manager provides strengths-based case management services to individuals living with HIV/AIDS. This role assists clients in accessing medical care, support services, insurance coverage, housing resources, transportation, and other community resources that promote health and well-being.
Responsibilities:

Conduct comprehensive client assessments and develop individualized service plans.
- Assist clients in accessing HIV medical care and support services.
- Provide referrals and linkage to community resources, including housing, food assistance, transportation, mental health services, and benefits programs.
- Monitor client progress and maintain ongoing contact to support retention in care.
- Advocate for clients and help address barriers to treatment and services.
- Maintain accurate and timely documentation in accordance with program requirements.
- Participate in interdisciplinary team meetings and case conferences.
- Ensure compliance with Ryan White, grant, and organizational requirements.
- Maintain client confidentiality and adhere to HIPAA regulations.
Requirements:

Bachelor’s degree required, master’s degree preferred.

Experience in case management, preferably with individuals living with HIV/AIDS.

Knowledge of HIV care, treatment, and available community resources.

Strong interpersonal, communication, and advocacy skills.

Ability to work independently and as part of an interdisciplinary team.

Excellent organizational and time-management skills.

Sensitivity to the unique challenges faced by individuals living with HIV.
