Position Summary
The Home Health Intake Coordinator is responsible for managing all incoming patient referrals and coordinating the admission process from referral receipt through Start of Care. This position serves as the primary point of contact for referral sources, patients, families, and clinical staff to ensure timely admissions, regulatory compliance, and excellent customer service.
Essential Duties and Responsibilities
- Receive, review, and process incoming referrals from hospitals, physicians, case managers, rehabilitation facilities, skilled nursing facilities, and insurance companies.
- Verify patient demographics, insurance coverage, eligibility, and authorization requirements.
- Review referral documentation to ensure completeness, including physician orders, face-to-face documentation, medication lists, and clinical records.
- Obtain missing documentation from referral sources, physician offices, and healthcare facilities as needed.
- Coordinate Start of Care visits and communicate admission details with patients, families, nurses, therapists, and agency leadership.
- Collaborate with the Director of Nursing and clinical team to determine acceptance of referrals based on patient needs, staffing availability, and service area.
- Enter and maintain accurate patient information in the electronic medical record system.
- Maintain referral logs and track referral outcomes, admissions, and conversion rates.
- Communicate referral status updates to referral sources and internal staff in a timely manner.
- Ensure compliance with Medicare, Medicaid, state, and agency regulations related to admissions and patient intake.
- Assist with intake-related audits, surveys, and quality improvement initiatives.
- Foster positive relationships with referral partners, physicians, discharge planners, and community organizations.
Qualifications
- Minimum of one 1 year of home health, hospice, healthcare intake experience required.
- Knowledge of Medicare, Medicaid, and home health regulations strongly preferred.
- Excellent organizational, communication, and customer service skills.
- Ability to prioritize tasks, work independently, and manage multiple referrals in a fast-paced environment.
Job Type: Full-time
Pay: $25.00 - $33.00 per hour
