Full-time Habilitation Coordinator
ARISE/Exceptional Family Resources, a growing Syracuse-based not-for-profit agency, is currently seeking a full-time Community Habilitation Coordinator to support people with developmental disabilities and oversee services from our Syracuse office.
Position Summary: Oversee the development and implementation of community habilitation services for people with developmental disabilities in Onondaga County. Supervise Direct Support Professionals (DSPs) provide community-based supports. Provide minimum of 15 hours/week of direct 1:1 or 1:2 support in the community.
Main Position Responsibilities:

Oversee community habilitation services, work closely with service recipients and their teams to ensure services meet the needs of the person

Develop and maintain Staff Action Plans and safeguards, train staff, ensure plans and safeguards are effectively implemented

Teach life skills, build natural relationships and explore new activities with the people receiving services

Support the growth of independent living skills such as cooking, shopping, healthy living, safety, and transportation

Promote and encourage social connections and community participation for the people we support by attending fitness classes, visiting local parks and festivals, joining a local organization or team, and more!

Provide behavioral support, communication support, and/or personal care assistance as needed

Follow and maintain scheduled days and times of service

Provide safe transportation in your personal vehicle as needed and requested

Follow guidance for staff support and safeguards

Maintain a minimum billing requirement of 15 hours/week
- Supervise and monitor the work of staff through on-site visits, feedback from service recipients and families served, and daily documentation

Facilitate introductions between potential staff and service recipients

Recruit, train, and develop 1:1 support staff

Ensure all services and documentation comply with Medicaid billing standards
Education, Experience, and Other Requirements:
- 1-2 years related experience working with people with Developmental Disabilities preferred

Familiarity with OPWDD requirements and oversight of disability services preferred

Reliable transportation required

Current auto insurance, valid NYS driver's license, and acceptable driving record required

Knowledge of computer systems and usage preferred (Microsoft Word, Excel, Publisher)

Must have technology available to download multiple apps for use in the community and abide by Agency's technology policy.
