Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health by taking vital signs such as blood pressure, pulse, and temperature; report changes to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and laundry
- Observe and document patient conditions, behaviors, and progress to inform ongoing care plans
- Use specialized equipment like Hoyer lifts or resident’s <PERSON> to safely transfer individuals with mobility challenges
Requirements
- Prior experience in home & community care, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation techniques, dementia and Alzheimer’s care, memory care, hospice care, and working with individuals with developmental disabilities or autism
- Certifications in First Aid and CPR are required; additional training in HIPAA compliance is a plus
- Ability to lift heavy objects safely and perform physically demanding tasks involving patient transfers or mobility assistance
- Strong communication skills to collaborate effectively with healthcare teams, residents, families, and other caregivers
- Compassionate attitude with a genuine desire to provide respectful and personalized care
- Flexibility to work various shifts including days, evenings, weekends, or holidays as needed
