You’ll serve as the face of our facility throughout the healthcare community by:
Building relationships with:
Hospital case managers
Social workers
Discharge planners
Physicians
Referral coordinators
Community partners
Helping families navigate transitions into skilled nursing care
Growing census through relationship-building and responsiveness
Coordinating closely with admissions and clinical teams
Visiting hospitals and healthcare partners regularly
Becoming the “go-to” person referral sources trust
Helping create smooth, positive patient transitions
Identifying opportunities to improve service and experience
Tracking referrals and following up consistently
This is a highly visible role with real impact on the success of the building.
$90,000–$100,000 base salary

SNF/LTC marketing or admissions experience
Existing hospital relationships in the Denver metro area
Experience with CarePort, Epic, PointClickCare, or other referral systems
Post-acute or healthcare sales background
