POSITION: Mental Health Coordinator-Bachelor's Level
SCHEDULE: Full-time/8am-4:30pm/Weekend off
FACILITY: Duffield Regional Jail
LOCATION: Duffield, VA
PAY RATE: Starting rate at $25.00 hr.

As a Mental Health Coordinator, you'll play an important role in identifying mental health needs, coordinating treatment, and helping ensure continuity of care.
Position Summary
As a Mental Health Coordinator, you'll serve as a key liaison between the healthcare team, correctional facility, community providers, and outside resources. Your responsibilities will include:
- Screening and triaging new admissions and referrals to identify mental health needs and appropriate levels of care
- Coordinating psychiatric and mental health services, including telepsychiatry clinics, medication needs, and follow-up care
- Collaborating with healthcare providers, security staff, community agencies, and supervision programs to support continuity of care
- Connecting patients with available services and coordinating discharge planning and community-based resources prior to release
- Managing patient requests, obtaining prior health records, and maintaining accurate documentation within the electronic health record
- Protecting patient confidentiality while supporting case management, reporting, and compliance with HIPAA, facility, and company requirements
You'll go beyond coordinating services.
You'll help bridge the gap between incarceration, treatment, and the community so patients have the support they need to move forward.

What You'll Need
- Bachelors Degree - human services field preferred
- QMHP credential
- Current CPR certification
- At least one (1) year prior clinical experience working with forensic clients; familiarity with relevant research literature, clinical assessments, procedures and methods, ability to assess treatment goals and write goal-directed, individualized treatment plans; ability to monitor and document individual behavior patterns and modify treatment plans; familiarity with diagnostic nomenclature outlined in the most recent Diagnostic and Statistical Manual. Correctional experience preferred.
