Nurse Practitioner

Essential job duties and responsibilities
We are looking for Nurse Practitioners to join our team on a Part-time or Full-time basis. The responsibilities will be, amongst others:
- Practice primary care through office & virtual visits. The role may include caring for residents at independent and/or assisted living facilities, based on availability and preference.
- Help to develop the care team (Registered Nurses, Medical Assistants, and others) that collectively manages a panel of patients.
- Appropriately order diagnostic studies, interpret results, and create individualized treatment plans using evidence based medicine.
- Manage medical and surgical conditions with knowledge of indications, contraindications, and adverse effects of treatment modalities.
- Counsel and educate patients and families.
- Perform accurate and timely documentation in the electronic medical record.
- Be a role model, mentor and coach for all others on the team.
- Perform other related duties as requested, and practice within the full scope of a Nurse Practitioner.
Qualifications

The ideal candidate has a strong affinity or connection with the populations we serve (Medicare, including patients with both Medicare and Medicaid.) Key criteria include:
- Leadership skills, passion for teamwork, ability to coach/mentor team members in a young, dynamic organization
- Master's degree and/or equivalent and successful completion of accredited nurse practitioner program (Adult Gerontology Nurse Practitioner or other)
- Unrestricted license to practice as a Nurse Practitioner in DC
- CDS and DEA for DC
- Relevant experience (working with elderly patients, lower-income patient and/or palliative care) preferred
- Enthusiastic to work in a performance-driven organization (data-driven, focused on continuous improvement of patient experience, care quality, and outcomes)
- Experience with Electronic Medical Records and office software
- Active driver’s license and reliable transportation

Pay: $110,000.00 - $150,000.00 per year
