1. Position Summary and Responsibilities:
· Under the supervision of the Registered Nurse, the Licensed practical nurse assesses, plans, implements and evaluates the needs of the rehabilitation patient for an assigned shift.
· Providing personal care, including bathing and dressing patients.
· Performing assessments and monitoring the physical and neurological condition of the patient
· Monitoring patients' conditions, including their eating habits and bathroom usage.
· Monitoring patients' vital signs, including temperature, blood pressure and heart rate.
· Maintaining patients' health records and ensuring scheduled shift documentation is completed
· Providing and managing clinical care, including scheduled and new medication orders, implementing orders for tests and procedures (taking blood samples, starting IVs, preforming EKGs and wound care) collaborating with doctors and members of the therapy team
· Administering medications, assessing efficacy and documenting appropriately
· Educating patients and their families about treatment plans and other aspects of their care.
2. Position Education / Qualifications:
· Associate’s degree (A.A.) or equivalent from two-year college or technical school
· Licensed Practical Nurse issued license through the State of Louisiana
· BLS required/ ACLS preferred

B. Physical Requirements:
· The position requires physical work but not to exceed 50 pounds. Any work load exceeding 50 pounds will require assistance. The individual will be required to pull up or reposition patients in bed, lift supplies, place equipment on supports, push beds throughout the hospital, and perform external cardiac compressions. The individual must be able to bend over to perform certain duties. The individual must also be able to maneuver through halls, stairways and patient rooms during their shift and in response to hospital emergencies. In the event there is a need to evacuate the building, heavy lifting will be required to carry patients to safety. A significant amount of writing is also required.
· Working Conditions: The individual spends over 95% of their time in an air conditioned environment with varying exposure to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have heavy exposure to infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
· Personal Protective Equipment: This position requires exposure to infectious wastes such as blood and body fluids that mandate the wearing of gloves, masks, gowns and goggles for every actual or potential exposure.
