Eprine Home Care seeks a State of New York Registered Nurse to work in its expanding Home Care program with seniors with Alzheimer's/ Dementia. Responsible for supervision of Aides, trainings and assesments of Patients.
Responsibilities:
1. Performs admission assessment within 24 hours of referral or patient’s return from the hospital
2. Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
3. Develops a written plan of care for each patient assigned semi-annually.
4. Assesses and reassesses patients for pain. Utilizes appropriate pain management.
5. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
6. Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.
7. UAS training
Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate
