We're growing! Join our team as a Pathways to Success Care Coordinator and help expand services to children/families who have complex behavioral health needs and require intensive services and support. Using a child and family team wraparound planning process, Pathways Care Coordination makes a real difference for youth and families by developing a family vision, identifying their needs, and connecting them to needed services.
With a manageable caseload of 10-16 children, you'll have the time to build strong, positive, working relationships with children, families, and other service providers.
This position requires:
- BA/BS degree in Psychology, Social Work, or other Human Services field
- Valid driver's license, reliable vehicle, and a good driving record
- Ability to pass fingerprint background checks and drug test
- Work in natural settings such as homes, schools, and/or in the community in Adams County and surrounding counties
- Employee residence within 30 miles of Quincy, IL
One year experience as a Mental Health Professional is preferred and experience working with children/adolescents is highly preferred.
Full-time position! Hours of work are primarily daytime, Monday - Friday, with two evenings/week and schedule flexibility to meet with children/families at times that work for them. This results in Care Coordinators being able to have flexible work schedules based on their personal or family needs too!
This is both a work-from-home and a community-based position where you will meet with your clients in community-based settings and perform office-type work from home.
This position isn't required to participate in our after-hours on-call rotation; rather, you would respond if a child on your caseload has a crisis during your work hours or, if after-hours, may be called upon for consultation about the child.
