Role Description:
Are you a dedicated and skilled Occupational Therapist looking to make a meaningful impact? We’re hiring for 1099 or W-2 roles in a home health capacity, where you'll provide services in the homes of patients on your caseload within your defined geographic area, with some opportunities for telehealth. This role involves evaluating and assessing patients' occupational therapy needs, crafting and implementing treatment plans to improve daily living skills, and documenting patient progress.
Pay Rates:
1099 Roles
- In-person therapy: Starting at $90/hour (ongoing), $150/visit (evaluation).
- Teletherapy: Starting at $65/hour (ongoing), $120/visit (evaluation).
W-2 Roles
Pay rates highly based on benefits package selected.
- In-person therapy: Starting at $70/hour (ongoing), $140/visit (evaluation).
- Teletherapy: Starting at $60/hour (ongoing), $115/visit (evaluation).
Additional Opportunities:

Requirements:
- Graduate from an accredited Occupational Therapy program.
- Hold an active or eligible Occupational Therapy license in Arizona (by start date).
- Current CPR Certification, valid Fingerprint Clearance Card, and Article 9 Training (support available to complete).
Essential Functions:
- Evaluate and treat a diverse caseload, including Autism, Down Syndrome, Cerebral Palsy, Cognitive Disabilities, and other genetic or chromosomal conditions.
- Document SOAP notes, progress reports, and evaluation reports using an intuitive EMR system.
- Travel from home to home within your defined geographic area to provide services to patients on your caseload.
