Advocates for Self-Determination, LLC is seeking an experienced and dependable SIP/CLS Team Lead to provide frontline leadership to Direct Care Workers supporting adults receiving Supported Independent Program (SIP) and Community Living Support (CLS) services.
This position is ideal for an experienced Direct Care Worker, Senior DCW, or Team Lead who is ready for greater responsibility and leadership development but still enjoys being directly involved with clients and staff.
The SIP/CLS Team Lead serves as the day-to-day connection between Direct Care Workers and management, helping ensure quality services, accurate documentation, effective implementation of Individual Plans of Service (IPOS), and a person-centered environment that promotes independence and client choice.
What You’ll Do
Lead & Support Your Team
- Provide day-to-day guidance and support to Direct Care Workers.
- Help staff understand shift responsibilities and client-specific expectations.
- Model professional communication and appropriate client interactions.
- Conduct and document routine staff coaching.
- Identify attendance, performance, conduct, and training concerns and communicate them to management.
- Assist with new-hire orientation and client-specific training.
- Help coordinate staffing coverage and communicate staffing needs.
- Reinforce company policies, procedures, and chain-of-command expectations.
Support Client Independence & IPOS Goals
- Maintain working knowledge of assigned clients’ IPOS, goals, Crisis Plans, supervision requirements, behavioral supports, health needs, and preferences.
- Ensure staff properly implement client IPOS requirements and goals.
- Promote independence, dignity, choice, and community inclusion.
- Monitor implementation of client goals and supports.
- Assist with coordinating appointments, activities, transportation, and community participation.
- Respect and advocate for client preferences regarding activities, routines, relationships, privacy, and community involvement.
- Report significant health, behavioral, safety, or functional changes promptly.
- Assist with preparation for case management, guardian, medical, and treatment-team meetings.
Documentation & Compliance
Strong documentation skills are essential for this position.
The Team Lead helps ensure required documentation is complete, accurate, organized, and submitted on time, including:
- CLS/service logs
- IPOS goal documentation
- Supervision/30-minute documentation
- Appointment Records
- Healthcare Chronology (HCC)
- Medication Administration Records (MARs)
- Community activity documentation
- Attendance records
- Mileage and transportation records
- Receipts
- Incident documentation
- Timesheets and timekeeping records
- Other required client and operational documentation
You will be expected to identify missing or inaccurate paperwork, follow up with staff, and escalate repeated documentation deficiencies to management.
Medication, Health & Safety
- Maintain required medication administration competency.
- Follow and reinforce the Eight Rights of Medication Administration.
- Assist with medication documentation audits.
- Immediately report medication errors, missed medications, refusals, or discrepancies.
- Ensure staff understand emergency procedures and client-specific medical protocols.
- Identify and report health and safety concerns promptly.
Operational Leadership
- Monitor the safety and condition of assigned service environments.
- Report and follow up on maintenance concerns.
- Assist with maintaining required supplies and client materials.
- Monitor company vehicle condition when applicable.
- Assist leadership with preparation for licensing, quality, Recipient Rights, case management, and other compliance reviews.
- Complete assigned operational responsibilities by established deadlines.
Communication
Our Team Leads are expected to be strong communicators.
You will:
- Respond to work-related communications during assigned working/on-call responsibilities.
- Communicate significant client and staffing concerns promptly.
- Keep management informed of unresolved concerns.
- Escalate emergencies immediately.
- Communicate professionally with clients, staff, guardians, case managers, and treatment-team members.
- Maintain client and employee confidentiality.
- Follow the established chain of command.
Qualifications
Required:
- High school diploma or GED.
- CHAMPS Provider ID#
- Dependable attendance and punctuality.
- Effective written and verbal communication skills.
- Ability to organize documentation and consistently meet deadlines.
- Ability to successfully complete required background checks, fingerprinting, training, and credentialing.
- Ability to maintain required CPR/First Aid and medication administration competency.
Preferred:
- Direct care, AFC, CLS, behavioral health, or developmental disability experience.
- Previous Team Lead, Senior DCW, supervisory, or leadership experience.
- Experience implementing IPOS goals and behavioral supports.
- Experience with medication administration and MAR documentation.
- Experience coaching or training Direct Care Workers.
