We are seeking a compassionate and experienced Social Services Director to join our team at Highland Springs Post Acute. As a leader on our interdisciplinary team, you will play a vital role in promoting and protecting the rights and well-being of our patients and residents.
Job Summary:
As a Social Services Director, you will collaborate with our team to ensure that our patients and residents receive high-quality care and support. Your responsibilities will include:
Developing and implementing individualized care plans that meet the unique needs of each patient or resident
Conducting patient and resident assessments to identify social, emotional, and psychological needs
Providing education and support to patients, residents, and families to improve quality of life
Coordinating discharge planning activities and making referrals to appropriate agencies and services
Establishing relationships with community-based agencies and services to ensure seamless transitions and referrals

Requirements:
Bachelor's degree from an accredited school of Social Work
At least one year of supervised social work experience in a skilled nursing facility, working directly with individuals
Licensed in Social Work for the state of VA
