Position Expectations
Each Housing Liaison shall engage in housing related functions on a full-time basis. Priority responsibilities of the position shall include, but not be limited to, the following: a) Practice Housing First, harm reduction and other low-barrier service delivery models. b) Attend local Continuum of Care meetings related to coordinated entry/case conferencing. c) Outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing homelessness. d) Refer and transport (as needed) individuals to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits. e) Landlord recruitment and rapport building. Directly contact potential landlords and provide information and make referrals to their units. Follow up with landlords about referrals and problem solve tenant issues to maintain rapport. f) Receive referrals from Continuum of Care programs to assist individuals with housing stability. g) Obtain SSI/SSDI Outreach, Access, and Recovery (SOAR) certification and assist individuals throughout the SOAR process. h) Assist with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing options.”
Ongoing Activities (including but not limited to)
Attend local Continuum of Care meetings related to coordinated entry/case conferencing. Participate in state-wide Housing Liaison trainings and calls. Complete timely data collection and entry as required by DMH and your home agency. Landlord recruitment and rapport building. Directly contact potential landlords and provide information and make referrals to their units. Follow up with landlords about referrals and problem solve tenant issues to maintain rapport. Outreach and engage individuals experiencing homelessness as referred by your CoC. Connect eligible participants to permanent housing, benefits, and ongoing supports. Serve as a resource to case managers, clients, and agency administration regarding housing and local/state resources. Caseload Management Activates (including but not limited to) | Receive referrals from Continuum of Care programs to assist individuals with housing stability. Assist eligible participants with obtaining identification documents required for housing or employment. Refer and transport (as needed) individuals to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits. Provide assertive outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing homelessness. Assist with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing options. Reengage active and former participants who have become disconnected with services or who are at risk of returning to homelessness.
