Job Title: Director of Nursing
Job Overview: The Director of Nursing provides leadership and consultative guidance to ensure high-quality, person-centered outcomes are supported through medical expertise and effective cross-department collaboration. This role guides and strengthens the Care Management team by providing clinical insight, strategic direction and actionable recommendations for high-risk and medically complex individuals. In addition, this position oversees the Nursing Team, ensures consistent follow-through on medical interventions, and maintains full-circle communication across departments.
Essential Responsibilities:

Provides direct supervision and oversight of the Nursing Support team

Maintains clinical accountability and an outcome-focused approach and ensures interventions are effective and evidence- based, while adhering to quality standards.

Serves as a consultant and subject matter expert for complex nursing/medical situations, offering direct guidance to Nursing and Care Management teams.

Supports the development, implementation, and streamlining of policies and procedures related to the Nursing team and broader clinical team functions.

Collaborates closely with the Director of Behavioral Health and Clinical leadership to ensure alignment across clinical support services.

Supports Care Management teams in understanding and executing medical interventions and recommendations.

Facilitates collaboration between Nursing Team and internal departments, ensuring that communication, workflows, and procedures are consistent and efficient.

Participates in and/or initiate clinical team meetings to ensure coordinated care for individuals with significant medical needs.

Reviews discharge reports and other related clinical documentation, including but not limited to PSYCKES and RHIOS, in order to provide expert guidance and input using a holistic approach.

Conducts case reviews and audits for individuals requiring medical interventions, as per required processes.

Analyzes data trends related to medical outcomes to inform decision-making, identify risks, and propose improvements.

Participates in or lead the development and delivery of training related to medical supports, documentation standards, and best practices.

Ensures timely responses to all inquiries and requests, with a commitment to addressing and resolving issues within 24 hours to maintain efficient operations and employee/individual satisfaction.

Takes lead and ownership of assigned tasks and special projects within the teams and assist with coverage as needed.

Maintains an up-to-date Outlook calendar to accurately reflect work-related meetings, appointments, and availability.

Ensures compliance with all OPWDD regulations as well as Tri-County Care policies.

Participation in internal and external meetings and committees as needed.

Performs other related duties as assigned by supervisor, to support overall organization’s goals and objectives
Specific Knowledge, Skills, and Abilities:
- Strong knowledge of intellectual and developmental disabilities (IDD), chronic medical conditions, behavioral health, and social determinants of health

Experience in a Health Home, care management, or similar population health setting preferred

Ability to lead and support a team in a consultative, non-direct care environment
-
Strong communication skills, with the ability to translate medical information for non-clinical staff

Experience with audits, quality assurance, and regulatory compliance
-
Knowledge of person-centered planning and OPWDD/Health Home requirements

Proficiency with electronic health records and care management systems

Understanding of HIPAA and confidentiality requirements
-
Demonstrated leadership, organizational, and problem-solving skills
-
Ability to engage effectively with individuals with behavioral health needs and diverse populations

Commitment to ethical practice, professionalism, and cultural competence
Experience:
- Active Licensure in NYS as a Registered Nurse
-
Minimum of 5–7 years of OPWDD experience, preferred
-
Expertise in medical analysis, person-centered planning, and clinical documentation
-
3 years of previous experience at a management level, preferred
-
Experience working within multidisciplinary teams and regulated program environments
Essential Responsibilities:

Oversee regional nursing support specialists.

Develop policy and procedures related to execution of nursing support specialist roles.

Liaison between the nursing and other departments to ensure that policies and procedures are being streamlined.

Collaborate with the clinical team on an ongoing basis

Provide medical input and guidance to nursing, care management, and clinical teams

Oversee development of health related trainings

Conduct case reviews upon request

Construct/implement policies and procedures.

Participate in mandatory staff meetings.
