Social Services Assistant
Job Summary:
At ALIYA of Crestwood, we're bringing healthcare back to basics. We spotlight each individual's needs and build custom care plans that help our residents rise up and get back on their feet. As a Social Services Assistant, you will play a vital role in planning, organizing, and directing the overall operation of our facility's Social Services Department. You will work closely with our interdisciplinary team to ensure that the medically related emotional and social needs of our residents are met and maintained on an individual basis.
Responsibilities:
Review facility policies and procedures to assure compliance with state and federal regulations
Participate in developing and implementing social work policies and procedures
Participate in quality assurance interdisciplinary team meetings
Document progress in meeting the psychosocial needs of residents
Work with the interdisciplinary team and administration to promote and protect resident rights and psychological well-being
Complete social histories and psychosocial assessments for each resident
Ensure therapeutic interventions and support residents and their families in coping with transition and adjustment to a long-term care facility
Provide groups and clinical interventions as needed, and coordinate discharge planning process
