Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patient observations such as vital signs, mental status, and overall health condition using Electronic Health Record (EHR) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy
- Perform light housekeeping tasks like cleaning, laundry, and meal preparation to ensure a safe and comfortable environment
- Utilize medical terminology accurately when recording medical documentation and communicating with healthcare teams
- Assist with specialized care needs such as catheter care, Alzheimer's and dementia care, hospice support, and memory care
- Operate equipment like Hoyer lifts safely and assist with transfers to prevent injury for both client and caregiver
- Engage in patient care activities that promote emotional well-being and independence in the home setting
Qualifications
- Proven experience as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home health environments
- Knowledge of medical terminology, vital signs measurement, medication administration, and basic medical procedures such as catheter care and CPR certification
- Experience working with individuals with developmental disabilities, dementia, Alzheimer’s disease, or hospice needs is highly desirable
- Familiarity with EMR/EHR systems for accurate documentation of patient information
- Ability to work independently while following detailed care plans and adhering to HIPAA standards for confidentiality
- Strong communication skills to effectively collaborate with healthcare professionals and family members
- Valid certification as a Home Health Aide or equivalent credential; additional certifications in CPR or hospice care are a plus
