Case Manager Duties
Harvest In-Home Care Inc.
The Case Manager is responsible for coordinating client care, supporting caregivers, maintaining compliance, and ensuring clients receive high-quality services that meet their individualized needs.
Client Assessments & Care Planning
- Conduct initial client assessments in the home, hospital, facility, or community setting.
- Develop individualized care plans based on client needs and preferences.
- Review and update care plans as client conditions change.
- Ensure all required admission paperwork is completed.
Client Monitoring & Quality Assurance
- Conduct supervisory visits and wellness checks as required by agency policy.
- Monitor client satisfaction and quality of care.
- Address client and family concerns promptly.
- Document all visits, communications, and care plan updates.
Caregiver Management
- Match caregivers with clients based on skills, experience, and personality.
- Provide caregiver orientation to client-specific care plans.
- Conduct caregiver supervisory visits and performance evaluations.
- Address caregiver concerns and provide ongoing support.
Communication & Coordination
- Serve as the primary point of contact for clients, families, caregivers, and healthcare professionals.
- Communicate changes in client condition to appropriate parties.
- Coordinate services with physicians, therapists, hospice, and other providers when necessary.
- Report significant issues to the Office Manager and Administrator.
Documentation & Compliance
- Maintain accurate and timely client records.
- Ensure all documentation meets agency, state, and regulatory requirements.
- Complete incident reports and follow-up investigations.
- Maintain confidentiality and HIPAA compliance.
Crisis Management
- Respond to urgent client and caregiver concerns.
- Assist with emergency staffing situations when needed.
- Coordinate appropriate interventions for changes in client condition.
Daily Tasks

- Timely completion of assessments and supervisory visits

- Reduction in client complaints and service interruptions
- On call [1 weekend out of the month]
The Case Manager’s primary goal is to ensure every client receives safe, compassionate, and high-quality care while supporting caregivers and maintaining compliance with agency standards.
Pay: $20.00 - $25.00 per hour
