Full-Time | Leadership Position | Day Shift | On-Call Rotation
Havenfield at Chilton Senior Living is seeking a compassionate, positive, professional, reliable, proactive, and detail-oriented Resident Care Coordinator (RCC) to join our leadership team.
The ideal candidate is passionate about serving seniors, possesses strong knowledge of common healthcare concerns affecting older adults, and is committed to providing exceptional resident care. We are looking for someone who not only responds effectively to resident needs and changes in condition but actively works to identify concerns before they become larger issues.
What You’ll DoResident Care & Clinical Coordination
- Monitor resident health status and report changes in condition.
- Complete assessments for new admissions and re-admissions.
- Develop, implement, and update resident care plans.
- Coordinate physician appointments, follow-up visits, and transportation.
- Review and process physician orders, medication changes, and treatment updates.
- Conduct resident chart audits and maintain accurate documentation.
- Complete monthly resident progress notes and care plan reviews.
- Proactively identify healthcare concerns and communicate appropriately with physicians, families, and healthcare providers.
Medication Management
- Oversee medication administration systems (MAR/eMAR).
- Review and update medication records daily.
- Coordinate pharmacy communications and medication orders.
- Train and support medication aides on proper medication administration and documentation.
- Complete medication audits and psychotropic medication reviews.
- Ensure proper medication destruction and disposal procedures are followed.
- Works alongside caregivers and medication aides to provide direct resident care and assistance with activities of daily living as needed, ensuring quality care and adequate support for residents and staff.
Leadership & Staff Support
- Supervise and support caregivers and medication aides.
- Assist with onboarding and training new staff members.
- Provide guidance on resident care, transfers, lifts, treatments, and safety procedures.
- Participate in investigations, incident reviews, and corrective action planning.
- Lead care plan reviews and resident care meetings.
- Serve as a resource and mentor to direct care staff.
Compliance & Documentation
- Ensure compliance with Wisconsin CBRF regulations.
- Maintain resident records and required tracking logs.
- Monitor incident reports and implement corrective actions.
- Protect resident rights, dignity, and confidentiality.
- Support facility preparedness during surveys and inspections.

- Proactive approach to resident wellness, prevention, and early intervention.
- Ability to maintain confidentiality and demonstrate professionalism at all times.
QualificationsRequired
- Minimum 2 years of experience as a caregiver, medication aide, RCC, CNA, or similar healthcare role.
- Strong knowledge of assisted living, CBRF, RCAC, memory care, or long-term care operations.
- Experience with medication administration and electronic documentation systems preferred.
- Excellent organizational, communication, and leadership skills.
- Ability to prioritize multiple responsibilities in a fast-paced environment.
- Proficient with computers, email, Microsoft Office, and electronic charting systems.
- Must be dependable and available for on-call responsibilities as scheduled.
- Ability to read, understand, and implement Wisconsin assisted living regulations.
Preferred
- Previous RCC, Lead Caregiver, Medication Aide, CBRF, RCAC, Memory Care, or Assisted Living leadership experience.
- Current Wisconsin CBRF certifications.
Physical Requirements
- Ability to lift, push, pull, and transfer up to 80 pounds.
- Ability to assist residents with transfers and activities of daily living as needed.
- Exposure to resident care environments and bodily fluids may occur.
