A $4,000 hiring bonus is being offered for this position. Hiring bonuses shall only be for new hires of original employment or re-employment as defined in Human Resources Policy No. 5.0.
$1,000 will be awarded in the first paycheck.
$1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.
$1,750 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.
The City of Virginia Beach Department of Human Services is seeking to hire several talented Clinicians who have experience providing intense case management services to consumers diagnosed with mental health and/or substance abuse in a Behavioral Health setting for our BH Case Management program.
Responsibilities:
Perform comprehensive and complex clinical functions including assessments, treatment planning, crisis planning, clinical intervention, consultation, supportive counseling, transition planning, and other case management duties in mental health and/or substance abuse settings.
Serve a specialized population of high-risk individuals who may be criminal justice-involved, have a history of frequent hospitalizations, experience homelessness or unstable housing, and present with multiple complex case management needs.
Providing assertive outreach, weekly contact, and intensive support to promote stabilization and connection to services to clients are often difficult to engage.
Provide community outreach in non-traditional centers such as homeless encampments, jails, hospitals, MAT clinics, and shelters.
Conduct risk assessments, developing individualized service plans, coordinating care across systems, engaging in community-based outreach, providing psychoeducation, and monitoring treatment progress.
Provide clients with enhanced care coordination with equal focus on both behavioral and primary health.
Ensure timely and accurate completion of clinical documentation and billing,
Serve additional specialized populations or roles.
Provide medically hospitalized clients with support with discharge planning, link with hospital staff and natural support as appropriate, and obtain medical records to ensure timely follow-up and quality coordination of care.

Minimum Requirements:
MINIMUMS: Requires a four (4) year degree in a human services field (such as psychology, behavioral or mental health, special education, social work) and three (3) years experience in fields providing the required knowledge, skills, and abilities and associated with such positions as Mental Health Counselor, Special Education Teacher, or Social Worker OR any combination of education (above the high school level) and/or experience and training equivalent to seven (7) years in fields providing the required knowledge, skills, and abilities. Must have, or obtain, a current and valid drivers license.
SPECIAL REQUIREMENTS: Must be registered with the Virginia Board of Counseling as a QMHP-A and/or Licensed or Registered as a Supervisee in Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Clinical Psychologist (LCP). All employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies.

Preferences:
Experience providing Medicaid reimbursable services to adults with High acute SMI/SUD.
Experience providing care coordination services to adults with high acute SMI/SUD.
Experience working with individuals involved in the criminal justice system including those on probation, parole, Mandatory/Behavioral Health Court or recently released jail/correctional hospital system.
Experience conducting community outreach in non-traditional centers such as homeless encampments, jail, hospitals and shelters
Experience in crisis intervention, de-escalation techniques and coordinating emergency psychiatric care
Experience in assisting clients in accessing public health benefits, including Medicaid, SNAP, and disability income.
Experience in providing harm reduction services, motivational interviewing, and building rapport with clients who are resistant to services.
