Job Description:
Hours : Monday -Friday 8am -4:30pm, Every other weekend.
Location: Our Hinsdale Hospice Office - 119 E Ogden Ave
Provides telephonic support to patient referral sources to initiate patient admission.
Documents patient demographic information, condition, referral source, and other pertinent data in electronic medical records.
Dispatches nurses, such as Admissions Nurses or Hospital Liaison Nurses, to assess patients and begin the admission process.
Updates and checks nurse schedules frequently to ensure appropriate appointments are being scheduled.
Communicates information to and from nurses regarding patient status and updates patient records accordingly.
Coordinates with nursing staff to facilitate patient admissions and ensure timely service delivery.
Maintains effective communication with patients, families, and healthcare providers throughout the admission process.
Orders special equipment or supplies as requested by the nursing staff.
Tracks and updates pending patient admissions by contacting patients or referral sources to schedule nurse visits.
Participates in staff meetings, department meetings, in-services, and other related activities.
Attends educational and developmental seminars and meetings as deemed appropriate.
Creates daily reports and communicates new patient admissions and referral outcomes to relevant individuals.
Obtains consents or provides information to patients' family members as needed.
Performs other duties as assigned.
Knowledge, Skills, and Abilities:
Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
Knowledge of electronic medical record. [Required]
Ability to complete hospice Orientation and Training, as necessary. [Required]
Knowledge of intermediate to advanced application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software. [Required]
Knowledge of medical terminology. [Required]
Ability to think critically regarding admission process. [Required]
