The Licensed Practical Nurse provides direct nursing care and clinical support within an Intensive Outpatient Program (IOP) and/or Partial Hospitalization Program (PHP). The LPN collaborates with a multidisciplinary treatment team to support clients experiencing mental health and/or substance use disorders. This role focuses on medication management, patient monitoring, education, and coordination of care in a structured outpatient setting. This is a full-time/in person role. Hours are M-F 9:00 AM- 4:00 PM
Key Responsibilities Clinical Care & Patient Monitoring
- Conduct nursing assessments, including vital signs, admission labs, withdrawal monitoring (e.g., CIWA, COWS), and general health status
- Observe and document patient behavior, symptoms, and progress in accordance with program protocols
- Report changes in patient condition to RN, physician, or clinical leadership promptly
Medication Management
- Administer medications within scope of practice
- Organize client pill boxes
- Monitor patients for side effects, adverse reactions, and medication compliance
- Maintain accurate medication records and documentation per regulatory standards
- Assist with medication reconciliation and patient education on prescribed treatments
Care Coordination
- Collaborate with therapists, case managers, and medical staff to support individualized treatment plans
- Assist with coordinating medical and psychiatric services as needed
- Communicate effectively with patients, families (when appropriate), and care team members
Documentation & Compliance
- Maintain accurate, timely, and compliant clinical documentation in the electronic health record (EHR)
- Adhere to all federal, state, and accrediting body standards (e.g., HIPAA, Joint Commission, CARF)
- Participate in audits, chart reviews, and quality improvement initiatives
Patient Support & Education
- Provide education on medications, health conditions, and coping strategies
- Support patients in understanding treatment goals and maintaining engagement in care
- Assist with crisis intervention and de-escalation when necessary
QualificationsRequired
- Current, active Licensed Practical Nurse (LPN) license in Georgia
- Minimum of 1 year nursing experience (behavioral health or substance use preferred)
- Basic Life Support (BLS/CPR) certification
- Strong documentation and communication skills
Preferred
- Experience in mental health, addiction treatment, detox, IOP, or PHP settings
- Familiarity with withdrawal management protocols (CIWA, COWS)
- Experience with electronic health records (EHR)
