ROLE PURPOSE
This is an office position, Monday - Friday 8am - 5pm.
The Clinical Care Coordinator provides assistance to the Agency Manager with supervision of clinical care team staff to ensure operational efficiency, productivity and compliance with agency policy and procedures.
DUTIES:
- Provides assistance to Care Team Leader/Clinical Manager with supervision of clinical care team staff and acts as a leader of the clinical care team. Maintains a committed and positive attitude, promoting teamwork and harmony by readily assisting team members and customers. Communicates effectively with all patients, families and physicians, as needed.
- Routinely communicates with and seeks guidance from Clinical Manager regarding patient/physician/clinical care team needs and identified issues. Provides report to the Clinical Manager as requested.
- Assists the Care Team Manager to ensure that all referrals within their service area are processed per agency policy. Provides back up services to intake personnel.
- Assists intake staff and the OASIS team with procurement of compliant F2F documentation for SOCs.
- Assists Manager with ensuring that all scheduled patient visits are covered by appropriate personnel. Assists schedulers with management of scheduling issues/conflicts as they arise.
- Tracks and communicates patient lab/test results to the physician and clinical care team. Ensures these priority documents are scanned into the EMR promptly.
- Assists QA department with identification of patients requiring specific QA oversight, as requested.
- Assists with the procurement of patient medical supplies/equipment. Works closely with clinical support staff to ensure medical supplies are readily available to clinical staff.
- Assists the Manager with management/review of the visit notes within the EMR
QUALIFICATIONS:
- Must hold a valid LVN License
- 1 year experience in Home Health; office setting preferred
- Wellsky experience helpful
- Must be willing to take Admin on call as needed
