Fox Chase Home Care is seeking a compassionate, organized, and bilingual (Spanish/English) Patient Care Coordinator to help patients and families navigate home care services while ensuring high-quality, coordinated care.
This role is ideal for someone with home care, healthcare, or care coordination experience who thrives in a fast-paced environment and enjoys building relationships, solving problems, and supporting patients through every stage of care.
As a Patient Care Coordinator, you will serve as a key liaison between patients, caregivers, families, Managed Care Organizations (MCOs), and internal teams to ensure services are coordinated efficiently and patients receive the support they need.

What You’ll Do
Patient Care Coordination
- Manage a caseload of home care patients and ensure services align with care plans and authorizations
- Coordinate communication between patients, caregivers, families, MCOs, and healthcare providers
- Conduct assessments to identify patient needs and service eligibility
- Develop and monitor individualized service plans to support patient well-being and continuity of care
- Identify and resolve gaps in care proactively
Patient & Family Support
- Serve as a primary point of contact for patients and families
- Educate patients on available services, resources, and care options
- Assist patients with navigating healthcare systems, community resources, and support services
- Build trusting relationships through compassionate communication and follow-up
- Utilize engagement and motivational techniques to improve patient participation and satisfaction
Compliance & Documentation
- Maintain accurate and timely documentation in accordance with state regulations and agency standards
- Ensure patient files, authorizations, and service records remain compliant and audit-ready
- Assist with quality assurance, compliance tracking, and reporting processes
Advocacy & Problem Resolution
- Respond to patient concerns and help resolve service-related issues quickly and professionally
- Advocate for patient needs while coordinating with internal departments and external partners
- Provide support during urgent situations and help coordinate appropriate interventions when necessary
What We’re Looking For
- Bilingual English/Spanish REQUIRED
- 2–4+ years of experience in patient care coordination, case management, home care, healthcare, or member services REQUIRED
- Strong understanding of home care workflows, MCO processes, patient authorizations, and care coordination
- Experience working with diverse populations and community-based resources
- Knowledge of behavioral health, social services, or healthcare navigation is a plus
- Excellent communication, organizational, and problem-solving skills
- Ability to manage multiple cases and priorities in a fast-paced environment
- Compassionate, patient-focused, and highly organized

Pay: $19.00 - $26.00 per hour
