Position Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing and long-term care environment under the supervision of a Registered Nurse or physician. The LPN is responsible for implementing individualized care plans, administering medications and treatments, monitoring residents' health status, documenting care, and collaborating with the interdisciplinary healthcare team to ensure high-quality, compassionate care for veterans and other eligible residents.
Essential Responsibilities
- Deliver direct nursing care in accordance with physician orders, nursing standards, and individualized care plans.
- Assist the Registered Nurse in assessing residents' health status and reporting changes in physical or mental condition.
- Administer prescribed medications, injections, IV therapies (within scope of practice), and treatments accurately and safely.
- Monitor residents for medication effectiveness and adverse reactions, reporting concerns promptly.
- Perform routine nursing procedures, including wound care, catheter care, specimen collection, and dressing changes.
- Obtain and document vital signs, weights, intake and output, and other clinical observations.
- Maintain complete, accurate, and timely documentation in the electronic medical record.
- Collaborate with physicians, Registered Nurses, rehabilitation staff, dietary personnel, social services, and other interdisciplinary team members to coordinate resident care.
- Respond to medical emergencies and provide nursing interventions within the LPN scope of practice.
- Educate residents, families, and caregivers regarding medications, treatments, disease processes, and health maintenance.
- Assist with admissions, transfers, and discharges, ensuring continuity of care.
- Supervise and provide guidance to nursing assistants and other direct care staff as assigned.
- Promote infection prevention practices and maintain a safe, clean, and therapeutic environment.
- Participate in quality improvement initiatives, mandatory education, and ongoing professional development.
- Maintain compliance with HIPAA requirements, state and federal regulations, and organizational policies and procedures.
Qualifications
- Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
- Graduation from an approved practical nursing program.
- Current CPR/BLS certification (or ability to obtain upon hire).
- Knowledge of long-term care nursing practices, medication administration, and geriatric care.
- Strong communication, critical thinking, and organizational skills.
- Ability to work collaboratively within an interdisciplinary healthcare team.
- Basic computer proficiency and experience with electronic health records preferred.
Work Environment
- Skilled nursing and long-term care setting serving veterans and eligible residents with acute, chronic, and rehabilitative healthcare needs.
- Shift work, including evenings, nights, weekends, holidays, and overtime, may be required.
- Frequent standing, walking, bending, lifting, repositioning residents, and providing direct patient care.
