Developmental Pathways (DP) is seeking Intake Case Managers to join our Case Management Team!
As an Intake Case Manager, you’ll play a vital role in delivering compassionate, person-centered service and support coordination to support access to Medicaid Waiver and State General Fund services. Your days will be filled with:
Conducting in-home visits to complete assessments and connect individuals to supportive programs and services.
Engaging with families and providers in the community to build trust and foster strong partnerships.
Collaborating at our Aurora office, sharing insights and strategies with your team and other departments to enhance service access.
Working from home, which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating intakes, and participating in virtual office hours, team meetings, and more.
Driving between locations, spending time in the community and at your home or chosen remote workspace. This dynamic, hybrid role offers a fast-paced and rewarding experience where you help weave together services that empower the lives of individuals with disabilities.
Schedule Enjoy a 4-day, 40-hour workweek (Monday–Thursday or Tuesday–Friday) with the flexibility to work remotely, in-office, and in the field.
This position requires frequent in-person presence to support essential responsibilities such as screenings, assessments, and other duties in alignment with state regulations and company policies. Candidates should be prepared to travel within our catchment area (Arapahoe, Douglas, and Elbert counties) on a weekly basis to fulfill these requirements. Flexibility and reliability in meeting these obligations are critical to success in this role.

Additional Responsibilities
Ensures timely response to internal and external referrals for individuals pursuing programmatic supports
Responsible for initial screening and assessment of individuals’ immediate and long-term needs and referring to appropriate internal and external programs and community resources to support assessed needs
Provides case management, eligibility determination support and waitlist support and updates to individuals waiting for services, as needed and applicable
Responsible for ongoing screening of individuals needs and referring to appropriate programs and community resources as appropriate
Ensures timely initiation and implementation of initial eligibility assessments and coordination process to access HCBS waiver services or other state-funded programs
Assists with Enrollment duties as needed
Assess the overall needs of an individual and assist with the exploration of programs, long-term services and supports, and other community resources to meet assessed needs
Assist individuals and families in maintaining eligibility through the completion of required assessments, monitoring, and support planning
Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies
Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
Schedule, facilitate, document, and complete follow-up for all required and ad-hoc meetings; disseminate updates to the necessary parties
Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to providers, individuals, and family members
Maintain a professional and ethical manner in all interactions and meet performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries
