LPN pay range-$25-$40 per visit
RN pay range- $35-$100 per visit

Job Responsibilities Include:
· Traveling to patient’s homes for Start of Care, Recertification, Discharge and Skilled Nursing Visits.
· Managing the Patient’s care and needs through home care visits.
· Implementing the plan of care in order to meet your patient’s needs.
· Audits and maintaining current and accurate patient records visits by electronically charting documentation in accordance with federal, state, and local policies/guidelines.
· Following current nursing professional standards of practice and agency approved protocols for home health care.
· Being a good resource for your patient and their family.
Requirements:
· Current Nursing License in the state of Ohio
· CPR/First Aid Certification
· Clean Background Check.
