Position Overview

Qualifications
- Active LPN license in Indiana
- At least one year of acute care experience or home health experience preferred
- New grads are welcome to apply
- Valid driver’s license and auto insurance
Key Responsibilities
- Provide in-home nursing care following physician and RN direction
- Monitor patient conditions and report changes
- Administer medications and educate patients and families
- Maintain accurate, timely clinical documentation
- Participate in team meetings and on-call rotation
Essential Functions
- Travel to patient homes within assigned territory
- Complete scheduled visits consistently and on time
- Document visits within 24 hours per guidelines
- Follow all state, federal, and company regulations
- Deliver care within LPN scope of practice
Pay: From $52,965.00 per year
