Responsibilities:
- Provide skilled nursing care to patients in their homes, following individualized care plans.
- Conduct comprehensive patient assessments and coordinate care with physicians, therapists, and other healthcare professionals.
- Act as a Case Manager, ensuring seamless communication between patients, families, and the healthcare team.
- Develop, implement, and evaluate personalized care plans to meet patient needs.
- Educate patients and families on disease processes, medications, and home care techniques.
- Ensure compliance with federal, state, and agency regulations.
- Assist with marketing efforts by developing relationships with referral sources, hospitals, and community partners to grow our patient base.
- Maintain accurate documentation and electronic health records.
Qualifications:
- Active RN license in Indiana or Compact License.
- Home Health and Case Management experience preferred.
- Marketing or business development experience is a plus!
- Strong critical thinking, organizational, and leadership skills.
- Excellent communication and interpersonal abilities.
- Valid driver’s license and reliable transportation.
