The St. Francois County Board for the Developmentally Disabled (SFCBDD) is seeking a compassionate, organized, and motivated Medical Care Coordinator to join our team. What You’ll Do
As the Medical Care Coordinator, you will work closely with our Registered Nurse and agency leadership to ensure individuals receive quality medical care and services in accordance with Missouri Department of Mental Health regulations and Home and Community-Based Waiver requirements.
Responsibilities include:
Medical Coordination
· Coordinate and schedule medical appointments for individuals receiving services.
· Attend medical appointments as needed.
· Organize and maintain medical records and documentation.
· Receive and process medical correspondence.
· Assist the RN with day-to-day medical coordination activities.
Documentation & Compliance
· Monitor completion of medical documentation to ensure accuracy and timeliness.
· Review medical-related documentation and incident reports.
· Assist with investigations and follow-up documentation.
· Prepare records and documentation for Department of Mental Health Licensure & Certification reviews.
Quality Assurance
· Conduct monthly residential home inspections.
· Monitor compliance with Missouri Department of Mental Health regulations, agency policies, and waiver requirements.
· Identify areas for improvement and assist with corrective action planning.
Staff Training & Support
· Provide training and coaching to direct support staff on medical procedures and documentation requirements.
· Participate in staff meetings, interdisciplinary team meetings, and administrative meetings as requested
· Serve as a resource to employees regarding medical-related concerns.
Communication
· Communicate effectively with physicians, nurses, pharmacists, families, guardians, case managers, and other professionals.
· Maintain positive working relationships with individuals receiving services and agency staff.
Additional Responsibilities
· Participate in the agency’s on-call rotation.
· Assist with agency projects and special assignments.
· Perform other duties as assigned.
Qualifications
We are looking for someone who has:
· High School Diploma or GED required -LPN Preferred
· At least one year of experience working with individuals with developmental disabilities
· Supervisory or management experience preferred
· Strong organizational and documentation skills
· Excellent communication and interpersonal abilities
· Ability to work independently while collaborating with a multidisciplinary team
· Missouri Class E Driver’s License with a good driving record
· Ability to successfully complete all required training, background checks, and health screenings required by Missouri Department of Mental Health regulations
Preferred Skills
· Experience with developmental disability services
· Knowledge of Missouri Department of Mental Health regulations
· Experience with Home and Community-Based Waiver programs
· Strong computer and electronic documentation skills
· Ability to prioritize multiple tasks in a fast-paced environment
· Attention to detail and strong problem-solving abilities
