Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health care team as a Case Manager. In this role, you will provide high-quality nursing care and case management services to patients in their homes. You will coordinate treatment plans and collaborate with physicians, caregivers, and interdisciplinary team members to ensure optimal patient outcomes.

Available Shift Options
Option 1: Pay-Per-Visit (Part-Time)
- Pay: $55.00 – $120.00 per visit
- Schedule: Flexible, based on patient needs
- Note: This is a pay-per-visit position (not hourly)
Option 2: Set Schedule (Part-Time)
- Schedule: Monday, Wednesday, Friday: 8:00 AM – 2:00 PM
- Pay: Up to $35.00 per hour
Option 3: Set Schedule (Part-Time)
- Schedule: Tues-Thurs: 8:00 AM – 2:00 PM
- Pay: Up to $35.00 per hour
Key Responsibilities
- Patient Assessment: Conduct comprehensive home health assessments, including physical, emotional, and social needs. Develop individualized care plans in collaboration with physicians and the care team.
- Care Coordination: Manage all aspects of patient care, including scheduling visits, obtaining medical orders, and ensuring continuity of care.
- Patient Education: Educate patients and families on conditions, medications, treatment plans, and self-care techniques.
- Ongoing Monitoring: Track patient progress, update care plans as needed, and communicate significant changes to the care team.
- Medication Management: Administer medications, monitor for side effects, and ensure compliance.
- Clinical Documentation: Maintain accurate and timely documentation in accordance with agency policies and regulations.
- Compliance & Safety: Follow all regulatory standards and ensure patient safety through risk identification and prevention.
- Family Support: Provide emotional support and guidance to patients and their families.
- Collaboration: Work closely with physicians, therapists, social workers, and other healthcare professionals to deliver coordinated care.
Qualifications
- Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program
- Licensure: Active and valid RN license in the state of Indiana
- Experience: Minimum of 2 years nursing experience; at least 1 year in home health or case management preferred
- Skills: Strong clinical assessment and critical thinking, Excellent communication and organizational skills. Ability to work independently and manage multiple patients. Proficiency in EHR systems and Microsoft Office
- Certifications: Current CPR certification required (additional certifications a plus)
Working Conditions
- Field-Based Role: Travel to patient homes within the designated service area, with some office-based responsibilities
- Physical Requirements: Ability to lift/assist patients and perform physical tasks such as standing, bending, and walking for extended periods
- Schedule Flexibility: May include occasional on-call or weekend coverage depending on patient needs
