Weekend Role Home Health & Hospice Intake High‑Impact Position
About the Role
We’re seeking a highly organized, compassionate Weekend Intake Coordinator to serve as the central connection point between referral sources, patients, families, and our clinical teams. This position is essential for ensuring timely, seamless care transitions during weekends and select weekday hours.

*** Candidates with HOME HEALTH, HOSPICE, HOME CARE or similar types of agencies will be GIVEN PREFERNCE.
***Ideal candidate will be located in one of the following markets: Flagstaff, Lake Havasu City, Cottonwood, Prescott, Phoenix, Casa Grande or Tucson***
What You’ll Do
Referral & Intake Coordination
- Process incoming Home Health & Hospice referrals from hospitals, facilities, physicians, and case managers.
- Collaborate with the Weekend Clinical Coordinator to support timely hospice evaluations and admissions.
- Verify insurance benefits and obtain authorizations as needed.
- Accurately enter and update patient information in the EMR (HCHB preferred).
- Track pending referrals and maintain organized hand-offs to weekday teams.
Communication & Customer Service
- Serve as the agency’s professional point of contact during weekend hours.
- Answer inbound calls, triage needs, and route urgent clinical issues appropriately.
- Provide clear, compassionate information to patients, families, and referral partners.
- Uphold compliance with all applicable home health and hospice regulations.
Culture & Conduct

What We’re Looking For
- 2+ years experience in intake, admissions, or related healthcare administrative role (home health or hospice strongly preferred).
- Proficiency with EMRs, referral portals, e‑fax tools, and Microsoft Office.
- Strong verbal/written communication skills and professional phone presence.
- Highly organized, detail‑oriented, and comfortable prioritizing in a fast-moving environment.
- Compassionate demeanor and patient-centered mindset.
- High school diploma required; LPN/LVN or similar credential preferred but not required.
Schedule
- Weekend daytime hours
- Limited Friday or Monday hours may be included based on operational need
- Remote or office-based (must have reliable internet/phone access if remote)

Pay: $25.00 - $28.00 per hour
