Home Care Intake Specialist $28.58/hour
Position Summary: This position is responsible for initial client assessments, service planning and coordination of care in compliance with AGE and agency guidelines within the 15 communities
serviced by Bristol Aging & Wellness, Inc.
Principal Accountabilities:

Assures that clients are assessed on a timely basis.

Conducts a comprehensive evaluation to determine the full scope of clients’ needs.

Determines eligibility for programs.
-
Develops appropriate service plans that meet consumer choice and budget guidelines.
-
Partners with vendors to implement home care services.
-
Assists clients with problem solving and makes referrals as needed.
-
Determines eligibility for public benefits.
-
Assists with applications for public benefits and health insurance and problem resolution related to benefits.

Documents phone calls, visits, and all activity in accordance with Elder Affairs’ and agency requirements.

Maintains client records accurately and within the mandated timeframe.

Communicate with case management staff upon a case transfer.
Qualifications:
Education: bachelor’s degree in human services or related discipline.
Experience: Elder service helpful.
