The Licensed Practical Nurse is responsible for providing quality care and meeting resident needs while following state guidelines and regulations. The nurses are also responsible for providing oversight during their shift to the Care Associates or other direct care associates as directed by the Executive Director or Wellness Director.
Reports to: MC Coordinator/ Executive Director
Essential Functions and responsibilities may include, but are not limited to the following:
- Ability to understand the needs of the residents.
- Ability to multitask and change programs if the needs of the residents require it.
- Ability to perform routine, repetitive tasks with frequent interruptions.
- Ability to understand facility’s fire and disaster plans; follow established procedures during drills and actual emergencies.
- Ability to participate in department quality improvement activities.
- Ability to perform/ execute the direct implementation of resident-based programming and activities in the community.
- Ability to provide training, supervision, and monitoring of associates in the administration of planned and unplanned activities with residents.
- Ability to perform ongoing assessment/observation of residents’ physical and cognitive needs and coordinates with the Wellness Director and Lifestyles Director to ensure personal service plans are up to date and accurate.
- Ability to promote resident physical and emotional health by assisting and teaching the resident regarding his/her medical care.
- Ability to observe resident conditions and accurately report to the Executive Director or Licensed Care Coordinators.
- Ability to provide personal care services which reflect the plan of service for each resident.
- Assists in keeping the environment safe for associates to reduce the occurrence of Workers’ Compensation claims by appropriately evaluating resident transfer and involving therapy services through resident wellness director and/or executive director.
- Assist the licensed Care Coordinator when needed with ongoing assessment/observation of residents' life enrichment needs and coordinates with other departments to assure quality, proactive care.
- Be knowledgeable of federal, state, and facility’s rules, regulations, policies and procedures.
- Comply with the facility’s policies and procedures.
- Comply with state regulatory guidelines.
- Evaluates residents; documents resident’s change in condition, and notifies resident wellness director, executive director, physician, and resident’s responsible party of those changes.
- Evaluate emergencies and determine actions per guidelines and policies.
- Ensure quality and safe resident care services.
- Foster teamwork with co-workers, take actions which assist others beyond job responsibilities, treat others with respect, and have a positive impact on co-workers, residents, and visitors.
- Perform other duties as assigned.
Education and Experience:
- Current license as an LPN in the state being employed.
- Leadership and Supervisory skills.
- Must be familiar with drug actions and interactions.
- Must possess accurate and comprehensive assessment skills.
- 3 to 6 months related experience preferred and/or similar training.
- Knowledge of senior living, Alzheimer's or a related dementia preferred.
- Per state requirements an active state certification & training.
Physical Demands and Working Conditions:
- This is a physically demanding job that could or does requires standing, moving, reaching, lifting, stretching, bending, and carrying things throughout the day.
- Position requires providing activities of daily living assistance to seniors and requires lifting and transferring or residents as appropriate.
- Must be able to lift, push, and/or pull 50lbs.
Knowledge and Skills:
- Ability to read and write comprehend simple instructions, short correspondence, and memos.
- Ability to communicate effectively in English with residents, family members and other associates
- Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
- Ability judgment in emergency situations and seeks assistance when needed.
- Ability to understand basic level of a skill and can follow written or verbal instructions.
- Ability to at times work flexible hours, such as early morning, evenings, weekends, and holidays.
- Ability to assist in emergency situations.
- Ability to understand facility’s fire and disaster plans; follow established procedures during drills and actual emergencies.
- Basic technical skills to complete a few repetitive and well-defined duties.
- Basic knowledge of the organization in order to answer basic questions.
- Basic knowledgeable of federal, state, and facility’s rules, regulations, policies and
- procedures.
- Follow defined safety codes while performing all duties.
- Follow defined Infection Control procedures.
Required Attendance:
- Attend scheduled department staff meetings.
- Attend in-service educational programs.
- Attend annual recertifications for state required license & certificates.
