Responsibilities:
Develop and implement social services programs to promote independence, quality of life, and early detection and intervention of disease management
Conduct assessments, develop care plans, and provide counseling services to residents and their families
Collaborate with interdisciplinary teams to ensure comprehensive care and services
Manage and supervise a team of social workers, counselors, and other social services staff
Develop and maintain positive relationships with residents, families, and healthcare providers
Ensure compliance with regulatory requirements and industry standards
Participate in quality improvement initiatives and strive for excellence in care delivery
Requirements:
Must-haves: Bachelor's degree in Social Work, Counseling, or related field; experience in social services or healthcare administration
Nice-to-haves: Master's degree in Social Work, Counseling, or related field; certification as a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC)
