Position Summary
We are seeking a compassionate and organized Qualified Mental Health Associate (QMHA) to serve as a Targeted Case Manager (TCM) and Psychosocial Rehabilitation (PSR) supporting Medicaid-eligible individuals across Nevada. This role is responsible for delivering person-centered case management services to individuals experiencing behavioral health challenges, developmental disabilities, or related conditions. The TCM helps clients access medical, psychiatric, housing, social, and educational services while ensuring compliance with Nevada Medicaid regulations.
Key Responsibilities
- Conduct comprehensive, strengths-based assessments of client needs and support systems.
- Develop and implement individualized service plans (ISPs) in collaboration with clients, families, and service providers.
- Coordinate access to physical and behavioral health care, housing, employment, and social supports.
- Assist clients in navigating Medicaid and other benefits programs.
- Provide advocacy to ensure timely access to appropriate services.
- Maintain regular contact with clients in accordance with Medicaid frequency requirements.
- Accurately document all services and contacts in the Electronic Health Record (EHR) per Medicaid and agency standards.
- Participate in interdisciplinary team meetings, case conferences, and supervision.
- Adhere to HIPAA, Nevada Medicaid, and internal compliance protocols.
Minimum Qualifications (As Required by Nevada Medicaid):
Candidates must meet one of the following criteria:
- Bachelor’s degree in a health- or human services-related field (e.g., social work, psychology, nursing, sociology),
OR
- Licensed as a QMHA. QBA may apply
OR
- Master’s degree in a behavioral health field (if under supervision of a licensed clinician).
Additional Requirements:
- Ability to pass FBI background check and maintain TB clearance.
- Valid Nevada driver’s license and reliable transportation.
- Strong organizational and communication skills.
- Experience with behavioral health or developmental disabilities preferred.
- Familiarity with Nevada Medicaid and documentation standards preferred.
Preferred Qualifications
- Bilingual (English/Spanish or English/Tagalog) strongly preferred
- Experience with Electronic Health Records (EHR) and Medicaid documentation standards
- Knowledge of Southern Nevada community-based resources
- Prior experience in mental health case management or social services
Working Conditions
- Work may be performed in-office, in the community, or remotely depending on client needs.
- Frequent travel for home visits, service coordination, or community meetings required.
- Caseload may vary based on funding and population served.
Job Types: Full-time, Part-time
Pay: $18.00 - $23.00 per hour
