One Home Health is seeking a Part-Time Occupational Therapist (OT) to join our growing Indiana team.

As a Home Health Occupational Therapist, you will help patients regain independence, improve daily functioning, and safely remain in their homes by:
- Performing comprehensive in-home occupational therapy evaluations
- Developing individualized treatment plans based on patient goals and clinical needs
- Implementing evidence-based therapeutic interventions to improve function and quality of life
- Educating patients and caregivers on safety strategies, adaptive techniques, and assistive equipment
- Monitoring patient progress and modifying treatment plans as needed
- Completing accurate and timely clinical documentation in accordance with agency and regulatory requirements
- Collaborating with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated patient care
- Maintaining compliance with all clinical, ethical, and professional standards
- Active Occupational Therapist (OT) license in the State of Indiana
- Current license in good standing
- Strong clinical assessment and decision-making skills
- Excellent communication and documentation abilities
- Ability to work independently and manage a flexible schedule
- Previous home health experience
- Experience working with adult and geriatric populations
