Your expertise in patient observation, assisting with Activities of Daily Living (ADLs), and delivering empathetic care will help us create a nurturing environment for those we serve. This paid position offers an opportunity to work closely with interdisciplinary teams, supporting patients through their final stages with professionalism and heartfelt compassion.
Responsibilities
- Deliver direct patient care by assisting with ADLs such as bathing, dressing, grooming, and mobility to promote independence and comfort.
- Monitor and document patient condition accurately, including vital signs, medication administration, and changes in health status using Electronic Health Record (EHR) systems.
- Support patients with specialized needs such as Alzheimer’s care, dementia care, memory care, and end-of-life hospice care to ensure safety and emotional well-being.
- Assist with medical procedures like catheter care, wound care, and basic first aid or CPR when necessary to maintain health standards.
- Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, meal preparation, and ensuring proper infection control practices in compliance with home healthcare regulations.
- Observe patient responses during visits and communicate effectively with family members and healthcare teams to coordinate ongoing care plans.
- Uphold patient privacy and confidentiality by adhering to HIPAA guidelines during all interactions and documentation.
Qualifications
- Valid certification as a Home Health Aide (HHA) or Certified Home Health Aide (CHHA) from an accredited program.
- Proven experience working in home healthcare settings or assisted living facilities; hospice or long-term care experience is highly preferred.
- Strong knowledge of medical terminology, medication administration protocols, vital signs monitoring, and dementia or Alzheimer’s disease care.
- Demonstrated ability to work effectively with geriatric patients and individuals with disabilities or developmental disabilities.
- Familiarity with EMR/EHR systems for accurate documentation of patient information.
- Ability to perform heavy lifting when necessary and operate assistive devices such as Hoyer lifts safely.
- Excellent communication skills coupled with compassionate caregiving qualities; ability to work independently while following healthcare guidelines.
- Valid CPR/First Aid certification; ongoing training in home & community care practices is a plus.
