Responsibilities
- Conduct thorough patient assessments (SOC and LTC) , including vital signs, physical examinations, and health history reviews, to develop individualized care plans.
- Utilize electronic health record (EHR) systemso accurately document patient interactions, treatment plans, and progress notes.
- Educate patients and their families on disease management, medication administration, nutrition, mobility aids, and health coaching strategies.
- Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to specialists such as cardiology or ophthalmology.
- Respond promptly to triage calls and urgent situations involving acute pain or infectious disease concerns while adhering to HIPAA regulations.
- Qualifications
- Valid RN license with current state licensure; additional certifications in Basic Life Support (BLS) required.
- Proven experience in home care nursing or related settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems.
- Clinical skills in IV insertion, phlebotomy, sterile processing, and specimen collection/processing (preferred), but not required.
- Experience managing diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia care needs.
- Knowledge of infection control practices and advanced skills in airway management and acute care procedures.
