Schedule: Day Shift | 5x8 Hours | 8:00 AM – 4:30 PM
Job Summary
"Navitas Healthcare, LLC" is seeking an experienced Physical Therapist for a Home Health assignment in the East Portland, OR area. This role is ideal for candidates with skilled home health experience who are comfortable working independently while providing high-quality patient-centered care in home settings.
Required Qualifications
- Minimum 1 year of Skilled Home Health PT experience required
- BCLS Certification required
- OASIS experience required
- First-time travelers accepted
- Must have reliable personal transportation
- Strong communication and patient care skills required
Key Responsibilities
- Provide physical therapy services to patients in home settings
- Evaluate patient conditions and develop individualized treatment plans
- Perform therapeutic exercises and mobility training
- Educate patients and caregivers on treatment plans and home exercises
- Maintain accurate documentation using EPIC and OASIS systems
- Coordinate care with interdisciplinary healthcare teams
- Monitor patient progress and adjust treatment plans as needed
