Tryfacta is seeking an
LPN Long Term Care (LTC) for our client in
Vestal, NY 13850. This is a

Position Title: LPN Long Term Care (LTC)
Location: Vestal, NY 13850
Duration: 16 Weeks
Work Schedule: Day/Evening/Night (5x8 and 3x12)
Pay rate:
$40 - $45
Responsibilities for this position include, but are not limited to:
- Administer medications and treatments as prescribed.
- Perform medication passes accurately and safely.
- Monitor and report changes in resident condition.
- Complete timely and accurate nursing documentation.
- Provide nursing care in accordance with facility policies.
- Supervise CNAs on assigned units.
- Provide care to residents with Alzheimer's disease and dementia.
- Support residents receiving skilled nursing, rehabilitation, memory care, and wound care services.

- Active New York State LPN license required at the time of submission.
- No license hits allowed.
- Minimum of 1 year of Long-Term Care (LTC) / Skilled Nursing experience required.
- BLS certification required.
- Must be able to pass a medication administration (med pass) test and clinical observation.
- Strong attendance and punctuality required.
- Must be dependable and able to work in a fast-paced environment.
- Must be flexible with scheduling needs.
- First-time travelers are accepted.
