Position Summary
The RN Clinical Manager – Hospice Intake & Admissions is responsible for the clinical and operational oversight of the hospice intake and admissions process. This position provides direct leadership and support to Intake Coordinators and Admission Nurses, ensuring that all referrals and admissions are processed accurately, timely, and in accordance with hospice protocols, regulatory requirements, and organizational standards.
The RN Clinical Manager serves as a key clinical resource for the intake and admissions teams and is responsible for reviewing medical records and clinical information to determine whether prospective hospice patients appear to meet hospice eligibility criteria. The position collaborates with referral sources, physicians, the Hospice Medical Director, patients and families, and the interdisciplinary team to facilitate appropriate and timely hospice admissions.
When needed, the RN Clinical Manager will provide direct clinical support in the field, including completing overflow or high-priority hospice admissions.
Essential ResponsibilitiesIntake & Admissions Oversight
- Oversee the daily activities and workflow of Intake Coordinators and Admission Nurses.
- Ensure intake and admission processes are consistently followed according to organizational protocols, policies, and regulatory requirements.
- Monitor referrals from initial receipt through clinical review, scheduling, admission, or appropriate disposition.
- Ensure referrals are handled promptly and appropriately based on patient needs and urgency.
- Monitor admission scheduling and workflow to minimize delays in initiating hospice services.
- Provide guidance and problem-solving support to Intake Coordinators and Admission Nurses when barriers or complex situations arise.
- Ensure effective communication and handoff between intake, admissions, and the ongoing hospice care team.
- Provide direct patient care and complete hospice admissions in the field when needed to support census, staffing, urgent referrals, or overflow.
Qualifications
- Current, unrestricted Arizona Registered Nurse (RN) license
- Minimum of 3 years of professional nursing experience; hospice experience required.
- Previous hospice intake, admissions, case management, or leadership experience preferred.
- Strong understanding of hospice philosophy and hospice eligibility principles.
- Demonstrated ability to review medical records and identify clinical indicators supporting potential hospice eligibility.
- Strong clinical judgment and critical-thinking skills.
- Excellent organizational, communication, and leadership skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Ability and willingness to perform field admissions and direct patient care when needed.
- Proficiency with electronic medical records and related healthcare technology.

Pay: $95,000.00 - $100,000.00 per year
