Facility Requirements:
- Must have BLS certification must be attached to submission packet (Must be through the American Heart Association)
- Must have mininum 4 years recent RN Case Management or Liscensed Clinical Social Worker experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, strong IDT collaboration
- Must have New Jersey RN or multi-state RN or NJ SW license (must be attached to resume)
- Shift is Monday - Friday 8a-430p
Duration - 13weeks
