Education/Requirements:
- Team oriented communication skills and ability to coordinate across a variety of departments for maximum efficiency and effectiveness
- Ability to conduct clinical and financial overview assessments of patients
- Ability to advocate for patients and facility by communicating with contracted health plans
Responsibilities
- Conduct comprehensive assessments of patients to develop individualized care plans.
- Coordinate with healthcare providers to ensure effective managed care and discharge planning.
- Maintain accurate and confidential patient records in compliance with HIPAA regulations.
- Provide education and support to patients and families regarding treatment options and resources.
- Facilitate intake processes and manage case documentation using EMR and EHR systems.
- Collaborate with interdisciplinary teams to optimize patient outcomes in various care settings.
