Position Summary
We are seeking a Registered Nurse (RN) Resource & Care Coordination Nurse to provide overnight care coordination, discharge planning, resource navigation, and clinical nursing support for the incarcerated population at a correctional facility located in Detroit.
This position blends traditional nursing responsibilities with care coordination and discharge planning functions. The nurse serves as an overnight resource for patients preparing for release while also providing clinical nursing support when operational needs arise.
The RN Resource & Care Coordination Nurse plays a critical role in ensuring continuity of care, reducing barriers to healthcare access, coordinating community resources, and supporting safe transitions back into the community. During periods of staffing shortages or increased clinical demand, this nurse will also assist with patient assessments, medication administration, and other direct nursing responsibilities.
Schedule
Full-Time Midnight Shift
Typical Schedule:
6:00PM – 6:30 AM
Rotating weekends and holidays as required.
Essential Responsibilities
Care Coordination & Resource Management

Coordinate continuity of care for patients preparing for release.
- Assist patients with access to community resources and support services.
- Facilitate referrals to:
- Primary care providers
- Behavioral health services
- Substance use treatment programs
- Housing and shelter resources
- Community-based support organizations
- Coordinate follow-up healthcare needs and appointments when appropriate.
- Collaborate with medical, mental health, and correctional staff to support comprehensive patient care plans.
- Identify barriers to successful community reintegration and assist with resource planning.
Transition of Care & Discharge Support

Assist with discharge planning and release preparation.
- Complete medication reconciliation for patients returning from outside appointments, hospitals, or preparing for release.
- Review discharge instructions and healthcare follow-up requirements.
- Ensure continuity of medications and treatment plans whenever possible.
- Support efforts to reduce gaps in care following release.
Clinical Nursing Support

Perform patient assessments as needed.
- Respond to urgent clinical situations.
- Assist with medication administration and medication passes when necessary.
- Provide nursing coverage during staffing shortages, call-offs, or periods of increased patient volume.
- Support intake, housing unit, and infirmary nursing operations as needed.
- Participate in emergency response activities consistent with facility protocols.
Documentation & Compliance

Maintain accurate and timely electronic medical record documentation.
- Complete required nursing and care coordination documentation.
- Ensure compliance with correctional healthcare standards, facility policies, and applicable regulatory requirements.
- Participate in quality improvement initiatives and interdisciplinary care planning.
Qualifications
Required

Current Michigan Registered Nurse (RN) license in good standing.
- Minimum two (2) years of nursing experience.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently and prioritize multiple responsibilities during overnight hours.
- Excellent communication, organization, and documentation skills.
Preferred
-
Correctional healthcare experience.
- Experience in case management, discharge planning, care coordination, or community health.
- Experience in emergency nursing, medical-surgical nursing, behavioral health, or public health.
- Knowledge of community healthcare resources and social service programs.
- BLS certification.
