- BS in Pharmacy (required).
- Current Louisiana pharmacist license (required).
- Hospital pharmacy experience preferred.
- Strong knowledge of drug therapy, contraindications, interactions, and USP <797> sterile standards.
- Ability to supervise pharmacy technicians and support personnel.

Provide safe, accurate, and cost-effective medication therapy management across the hospital, fulfilling physician orders, monitoring drug therapy, and supporting medication safety.
- Prepare and dispense medications according to physician orders and protocols.
- Interpret, verify, and document written and electronic medication orders.
- Maintain accurate patient medication profiles.
- Compound sterile products, chemotherapy, and TPN in accordance with USP <797>.
- Issue and document controlled substances per regulations.
- Monitor therapies for contraindications, interactions, allergies, and appropriateness.
- Review patient charts and document clinical interventions.
- Identify and report adverse drug reactions.
- Consult with practitioners and recommend cost-effective therapeutic alternatives.
- Participate in medication use reviews, audits, and quality improvement activities.
- Supervise pharmacy technicians and support personnel.
- Perform medication storage inspections and remove outdated medications.
- Support pharmacy automation and technology systems.
- Collaborate with IS, vendors, Nursing, Billing, and Clinical Informatics.
- Maintain licensure, CE documentation, competency requirements, and attend meetings.
- Perform all other duties as assigned.
