Position Summary
We are seeking an experienced and highly organized Director of Nursing (DON) to lead the clinical operations of our home health division. The DON is responsible for overseeing all nursing services, ensuring regulatory compliance, maintaining clinical quality standards, and providing leadership and mentorship to nursing and clinical support staff. This is a senior leadership role requiring a strong command of Pennsylvania home health regulations, Medicare/Medicaid conditions of participation, and CARF accreditation standards. The ideal candidate is a decisive clinical leader who thrives in a dynamic, community-based environment and is passionate about delivering excellent care to underserved and complex patient populations.
Key Responsibilities
Clinical Leadership & Oversight

Provide direct oversight of all skilled nursing services delivered under Title 28 (Chapters 601/611) home health licensure

Supervise and evaluate RNs, LPNs, home health aides, and other clinical staff; conduct performance reviews and corrective action as needed

Conduct or oversee clinical intake, start-of-care assessments, OASIS data collection, and plan of care development

Ensure appropriate physician orders, care coordination, and timely recertification and discharge planning for all active patients

Review and co-sign clinical documentation to ensure accuracy, completeness, and compliance with payer and regulatory requirements
Regulatory Compliance & Quality

Ensure ongoing compliance with Pennsylvania Department of Health regulations governing home health agencies (Title 28, Chapters 601/611)

Lead QAPI (Quality Assurance and Performance Improvement) initiatives, including tracking clinical outcomes, infection control, incident reporting, and adverse event review

Maintain agency readiness for Pennsylvania DOH surveys, CARF accreditation reviews, and managed care organization (MCO) audits

Oversee compliance with OLTL/Community HealthChoices waiver clinical requirements and EVV mandates
-
Develop, review, and update clinical policies and procedures in alignment with current best practices and regulatory standards
Staff Development & Training

Lead orientation and ongoing competency evaluation for all clinical staff including RNs, LPNs, HHAs, and DSPs

Identify training gaps and implement in-service education programs to address clinical and compliance needs

Foster a culture of accountability, continuous improvement, and person-centered care throughout the clinical team
Operational & Administrative Duties

Collaborate with the Associate Administrator and executive leadership on clinical strategy, staffing, budgeting, and service line expansion

Oversee clinical scheduling to ensure adequate staffing coverage and continuity of care for all patients

Serve as the primary clinical liaison with referring physicians, hospital discharge planners, MCOs, and community partners

Maintain working knowledge of Axxess or other EHR platforms used for clinical documentation and reporting

Participate in leadership meetings, board-level reporting, and strategic planning initiatives as needed
Required Qualifications

Current, unrestricted Pennsylvania Registered Nurse (RN) license in good standing
-
Bachelor of Science in Nursing (BSN) required; Master's degree in Nursing, Healthcare Administration, or related field preferred

Minimum 3–5 years of clinical nursing experience, with at least 2 years in a home health or community-based setting

Minimum 2 years of supervisory or nursing leadership experience

Strong working knowledge of Pennsylvania Title 28 home health regulations and Medicare/Medicaid Conditions of Participation

Experience with OASIS documentation, care plan development, and clinical quality metrics
-
Valid Pennsylvania driver's license and reliable transportation
-
Ability to pass Pennsylvania criminal background check, child abuse clearance, and FBI fingerprinting

Current CPR/BLS certification
-
Excellent written and verbal communication, organizational, and problem-solving skills
Preferred Qualifications
-
Experience serving as DON, Assistant DON, or Clinical Manager in a Pennsylvania-licensed home health agency

Familiarity with CARF accreditation standards and survey preparation

Experience with OLTL/Community HealthChoices or ODP waiver programs
-
Knowledge of Title 55 (Chapters 6100/6400) residential habilitation and IDD service frameworks

Proficiency with Axxess home health EHR platform (strongly preferred)

Experience with EVV platforms such as HHAeXchange or Sandata
-
Background in IDD, behavioral health, or dual-diagnosis populations

Work Environment
This is primarily an office-based leadership role at QHCG's Glenolden, PA headquarters, with periodic field visits to patient homes and residential sites for supervisory and quality assurance purposes. The DON must be available for on-call clinical consultation and regulatory response as needed.
