Saad Hospice is seeking an LPN who is looking for a change of pace in an office setting.
The position entails working in the office, answering and triaging calls from patients, caregivers, and employees.
Responsible for helping with the below:
DUTIES AND RESPONSIBILITIES
:
Responsible for reviewing and documenting on the communication log daily and following up with resolutions added into the log.

Answers after-hours phone calls, triaging symptoms via phone, communicating with the on-call team to assist with what is best needed for the Hospice patient.

Act as a resource for patient education and information, instructing patients/caregivers concerning treatments, test results, and other information as needed.

In coordination with the Clinical Director, they determine the urgency of seeing the patient based on brief assessment and on familiarity with a patient's condition.

Able to display professionalism when triaging emergent situations.

Coordinates with the holistic care team to facilitate efficiency of services.

Understands the needs of the organization and supports the mission, values, and management of patient care services.

Assists the Scheduling Coordinator with the Scheduling Duties assigned.

Pulls and logs IPU supplies as needed.

Helps coordinate transfer of a patient from the field to our inpatient unit for symptom management if necessary.

Helps the on-call team get visits scheduled as needed and assists in communicating with patients/families.
Salary compensable with experience.

Schedule: 8p-8a 3 to 4 day weekly rotation with every other weekend rotation.
