JOB SUMMARY:
The Clinic Nurse Navigator serves as a key point of contact for patients receiving care at
Rutland County Health Partners Free Care Clinic. This role provides clinical guidance, care
coordination, and patient education to ensure individuals—especially underserved and
uninsured populations—can effectively access and navigate healthcare services.
ESSENTIAL JOB RESPONSIBILITIES:
Patient Navigation & Care Coordination
- Guide patients through the healthcare system, including referrals and follow-ups, and access to services
- Coordinate care with external providers and community resources including but not limited to the YouFirst and Wise Women Programs
- Provide insurance navigation support including assistance with enrollment through Vermont Health Connect
- Deliver tobacco cessation support, including training and administration of Nicotine Replacement Therapy
- Complete additional patient navigation activities as required to meet grant objectives and reporting requirements
Clinical Support
- Perform nursing assessments and assist providers
- Administer vaccines per protocols and VDH requirements
Patient Education & Advocacy
- Educate patient on chronic disease management and preventive care
Community Outreach
- Connect patients with social services and community programs
- Attend community outreach events including those events involving the Mobile Health Unit
OTHER RESPONSIBILITIES
Administrative
- Assist the Executive Director, Medical and Dental Directors and others monitor and maintain the Quality Improvement Program
- Monitor clinical policies and procedures and provide updates and education to clinical volunteers
- Monitor clinical competence for clinical volunteers
- Assist with data collection and reporting
- Attend staff meetings and participate in program planning discussions.
- Support special projects and other administrative tasks as needed.
- Contribute to fostering an inclusive, supportive environment for patients, staff, and community members.
EDUCATION AND EXPERIENCE
Required:
- Active RN license in Vermont
- Minimum 2 years clinical experience
- CPR Certified
- Excellent written and verbal communication skills.
- Strong organizational skills and attention to detail.
- Ability to work independently and as part of a small, dedicated team.
Preferred:
- Experience in care navigation or community health.
- Familiarity with the needs of underserved communities in rural Vermont.
- Proficiency with Microsoft Office, social medical platforms, and basic website management.
WORK CONDITIONS
- Full-time: 32 hours per week
- Flexible schedule with some evening and weekend hours required.
- Approximately 4 hours per month on the Mobile Health Unit at community screening events.
- Valid driver’s license and reliable transportation required.
COMPENSATION & BENEFITS
- Non-Exempt: $31.00-$39.00 per hour commensurate with experience.
