Job Description:
· Assists patients and their families with ADLs and IADLs in their place of residence
· Responsible for working collaboratively with the Intake team, nurses, and with administrator in ensuring we uphold all applicable legal requirements, standards, policies, and procedures while taking excellent care of our patients. A very collaborate role!
Responsibilities:
· Functions as a member of the Hospice Interdisciplinary Team.
· Adheres to the highest standards of professional and personal conduct in the performance of his/her duties.
· Provides regular communication with the hospice staff regarding pertinent organizational and clinical issues.
· Oversees the accurate and timely completion of Medical Records including compliance with applicable laws and regulations.
Qualifications:
- This position requires at least 1 year paid experience as a CHHA in a health care facility or Home Health setting in the last 24 months.
- Candidates must have a current certification by the California Department of Health Services as a Home Health Aide, current California driver's license, access to an operating motor vehicle with at least minimum insurance.
- Valid BLS/CPR approved by the American Heart Association is required.
- Computer literate
- Possess knowledge of Hospice regulations and procedures
